NEW Highway Restrictions in NYSNEW HIGHWAY RESTRICTIONS EFFECTIVE NOVEMBER 15, 2008 Effective November 15, 2008, the Department intends to begin issuing oversize/overweight permits that include select highway restrictions. This is part of the Department's efforts to implement a Large Truck Policy that includes considerations for the safety of the traveling public, and the protection of the highways and the environment. For this Policy, Large Trucks have been defined as tractor-trailer or truck-trailer combinations where the trailer length is 45 feet or longer. It is the goal of the Large Truck Policy to encourage carriers to utilize the national network (primarily the Interstate highway system) to the maximum extent practicable. (Updated On Availability)Indiana:Forms M-203 (here) and the Accord Certificate of Liability should be filled out by your insurance company and faxed to 1-317-821-2339. The certificate holder area of the Accord Form should read: Indiana Department of Revenue -------------------------------------------------------------------------------- Missouri:Beginning February 28, 2009 Missouri will require all OS/OW carriers to file an Accord Certificate of Liability with their office.
